1. Acknowledgment of Activity & Risks

I understand that participation in golf instruction and related activities involves inherent risks,
including but not limited to:

• Physical exertion and athletic movement

• Repetitive motion injuries (including back, shoulder, elbow, wrist, knee, and hip injuries)

• Being struck by golf clubs, golf balls, or training devices

• Use of driving ranges, golf courses, simulators, practice facilities, nets, mats, and training
  equipment

• Slips, trips, falls, or uneven surfaces

• Instruction provided in person, virtually, online, or through recorded/video analysis and social
  media platforms

I understand that these risks may occur even when reasonable care is exercised.

2. Assumption of Risk

I knowingly and voluntarily assume all risks, both known and unknown, associated with participation
in golf instruction, practice sessions, drills, demonstrations, fitness movements, and related
activities.

Participation is voluntary, and I choose to participate despite these risks.

3. Release of Liability (Including Negligence)

In consideration of being permitted to participate, I hereby release, waive, and discharge:

Stanley Golf LLC, doing business as Golf-Ed, including its owners, members, managers, instructors,
assistants, employees, agents, independent contractors, affiliates, successors, assigns, and any
facility owners or operators where instruction may occur,

from any and all claims, demands, or liability, including claims arising from negligence or fault of
the released parties, for:

• Personal injury

• Illness

• Disability

• Death

• Property damage or loss

arising out of or related to my participation in golf instruction or related activities, to the
fullest extent permitted under Florida law.

4. Indemnification & Hold Harmless

I agree to indemnify, defend, and hold harmless the released parties from any claims, damages,
losses, or expenses (including reasonable attorney's fees) arising from my participation, or the
participation of any minor for whom I am legally responsible.

5. Medical Fitness & Emergency Consent

I certify that I am physically able to participate and have not been advised otherwise by a qualified
medical professional.

I authorize emergency medical treatment if deemed necessary and accept full responsibility for any
related costs.

6. Minors (Participants Under 18)

As the parent or legal guardian of a minor participant, I consent to the minor's participation and
agree to all terms of this agreement on the minor's behalf, including the release and
indemnification provisions, to the fullest extent allowed by Florida law.

7. Virtual, Online & Social Media Instruction Disclaimer

I understand that instruction provided virtually, online, through video analysis, or via social media
is educational in nature only. Results are not guaranteed, and I am responsible for my own safety,
environment, physical limitations, and execution of movements when practicing remotely.

8. Photo & Video Release

I grant permission for photographs and video recordings taken during instruction to be used for
instructional, educational, marketing, or promotional purposes by Stanley Golf LLC d/b/a Golf-Ed,
without compensation.

9. Governing Law & Severability

This agreement shall be governed by and interpreted in accordance with the laws of the State of
Florida.

If any provision of this agreement is found to be invalid or unenforceable, the remaining provisions
shall continue in full force and effect.

10. Acknowledgment & Signature

I have read this agreement carefully. I understand that I am waiving important legal rights,
including the right to sue. I sign this agreement voluntarily and of my own free will.
